I would say it was drawn, then scanned, then the background was removed and with it some traces. Then some contrast adjustments and may be a filter but not necessarily.

wow scare , that's good.
It has a filter early in the process, and it's a scratch and draw then worked and re- worked, but you basically nailed it.
